Job summary

reach community services ltd. is seeking a Centre Manager (ICCP) to oversee integrated community care services across multiple regions in Singapore. The role includes managing workflows, staff development, and ensuring service quality and compliance with community care standards.

Qualifications

• Minimum 3 years of experience in care coordination or community care programme management.
• Experience in a supervisory or team leadership role.
• Managing funded programmes such as AIC / MOH / NCSS is required.

Responsibilities

• Oversee management and operational oversight of the ICCP portfolio.
• Coordinate and implement ICCP workflows and referral pathways.
• Maintain relationships with healthcare partners and community service providers.
