Job summary

ALLIED SEARCH PTE. LTD. is seeking an Executive (Integrated Community Care Provider) to lead the implementation of programmes and integrated care initiatives, including joint referrals and care coordination across sub-regions in Singapore.

You will conduct interRAI assessments, build partnerships with healthcare and social service organisations, and coordinate meetings to monitor programme progress and address gaps. Degree in Social Work, Nursing or healthcare-related discipline required.

Qualifications

• Degree in Social Work, Nursing or healthcare-related discipline is required.

Responsibilities

• Drive the implementation of programmes and work plans, including integrated care initiatives and care coordination activities.
• Conduct interRAI assessments and support care planning for clients.
• Build and manage partnerships with healthcare, community and social service organisations within assigned sub-regions.
• Represent the organisation in sub-regional initiatives and liaise between internal teams, external partners and community stakeholders.
• Coordinate meetings and operational activities, monitor programme performance and address gaps.
• Identify opportunities to improve service integration and quality, and support reporting, dashboards and programme oversight.
• Perform ad hoc duties as assigned.
