Job summary

Home Nursing Foundation seeks an Executive to coordinate ICCP programme activities and drive care coordination for elderly seniors across our sub-regions in Singapore. You will liaise with network partners, support governance meetings, and help align services to organisational goals.

The role emphasizes strong stakeholder management, InterRAI assessments, and monitoring of service delivery to identify improvements. You will report to the Head of Regional Services in a fast-paced environment.

Qualifications

Degree in social work, nursing or healthcare-related discipline would be an advantage.
Strong interpersonal communication and stakeholder management skills are essential.
Experience coordinating integrated care initiatives is a plus.

Responsibilities

Implement ICCP programme activities and workplans, piloting joint referral workflows and care coordination initiatives.
Conduct InterRAI care assessments and undertake care planning and coordination responsibilities.
Serve as primary liaison with partner organisations to enable service integration across sub-regions.
Represent HNF in ICCP sub-regions, aligning with organisational objectives and partnerships.
Support community outreach to raise ICCP awareness and collaboration with stakeholders.
Organise ICCP Steering Committee and operational meetings, prepare agendas, take minutes, and follow up actions.
Act as a key liaison between internal teams and external partners for clear communication and smooth execution.
Monitor service delivery performance and identify improvement opportunities.
Assist in preparing reports, dashboards, and presentations for management and external reporting.
