Job summary

IHH Healthcare Bhd is seeking a PCN Executive to support the implementation and performance of Primary Care Network (PCN) and Healthier SG (HSG) programmes in Singapore. The role focuses on data monitoring, programme coordination, patient engagement initiatives, and quality improvement projects to drive programme outcomes, stakeholder engagement, and MOH/AIC compliance.

Qualifications

• Degree in Healthcare Administration, Public Health, Life Sciences, Healthcare or a related field.
• 2–4 years of experience in healthcare operations, programme management, project coordination, or stakeholder engagement.
• Experience in data analysis, performance monitoring, and presentation preparation.
• Familiarity with PCN, Healthier SG, or Singapore healthcare programmes is an added advantage.
• Strong project management and change management experience will be beneficial.

Responsibilities

• Monitor programme performance and prepare reports and presentation materials for management updates.
• Drive, coordinate and implement initiatives to improve clinic participation and achieve PCN and HSG performance targets.
• Oversee patient engagement campaigns, including outreach efforts to encourage follow-up care and close care gaps.
• Lead and coordinate projects aimed at improving health outcomes, such as vaccination uptake and engagement of non-responsive HSG enrollees.
• Support programme planning, implementation, and stakeholder engagement across PCN and HSG initiatives.
• Ensure timely reporting, documentation, and compliance with MOH/AIC programme requirements.
