Singapore General Hospital Pte Ltd. is seeking a detail-oriented professional to plan and facilitate care integration projects under the Division Population Health & Integrated Care (PHIC) ICCO.

You will coordinate operational and administrative support ensuring smooth operations of PHIC programmes and liaise with SingHealth and external partners. Responsibilities include meetings, data collection for analysis, and supporting audits.

Qualifikationen

• Bachelor's Degree with at least 4 years working experience will be an advantage.
• Strong analytical skills with an understanding of financials, statistics and workflow engineering would be an added advantage.
• Highly independent yet effective in team setting.
• Good interpersonal skills.
• Adaptable and able to work with various stakeholders.

Aufgaben

• Facilitate projects/programmes under Division of PHIC ICCO, including planning, implementing and operationalizing activities to achieve key objectives.
• Engage and liaise with internal and external stakeholders including clinical and operational departments from SingHealth institutions and external partners.
• Monitor programme progress and provide regular updates to programme director and funder.
• Facilitate and support regular meetings including case discussions in multi-disciplinary meetings and minutes preparation.
• Coordinate data requirements and ensure timely submission of indicators and reports.

You are responsible for planning and facilitating care integration projects and activities driven out of the Division Population Health & Integrated Care (PHIC) Integrated Continuing Care Operations (ICCO). The main accountabilities includes providing and coordinating operational and administrative support to ensure smooth and effective operations of PHIC programmes.

Your job roles will include, but be limited to the following:

• Facilitate projects/programmes under Division of PHIC ICCO, which includes planning, implementing and operationalizing activities to achieve key objectives.

• Engage and liaise with internal and external stakeholders, including clinical and operational departments from SingHealth institutions, external community partners such as eldercare service providers, primary care partners and government agencies involved in the programme.

• Monitor programme progress and provide regular updates to programme director, senior management and relevant agencies/funder.

• Facilitate and support regular meetings including case discussion in multi-disciplinary meetings, administrative meetings, and preparation of minutes.

• Work with business units to understand data requirements and establish criteria to collect the relevant information for analysis and evaluation of programme. Ensure timely submission of indicators and reports to programme director and funder.

• Work with Finance to raise and process claims with the necessary documentation in accordance with the financial guidelines, processes and governance rules.

• Maintain necessary documentation for audits and serve as the main contact point for audit queries.

• Provide support to Div PHIC programmes as necessary to ensure efficient and effective operations.
