Job summary

Lions Befrienders Service Association (Singapore) is seeking a diligent Coordinator for Home Personal Care Services. The role supports programme operations, including frail and home-bound clients, with administrative duties and care coordination.

You will manage client admissions/discharges, maintain records for audit, and oversee invoicing, subventions, and reports. The position requires strong MS Office skills and a passion for eldercare, with monthly means-testing and stakeholder outreach

Qualifications

• Minimum 2 years of related experience in eldercare/social service or related sector.
• Minimum of a Higher Nitec in Business Administration, Office Management related.
• Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
• Passionate about working with elderly and willingness to learn.
• Good understanding of working with elderly.
• Meticulous and highly accurate in documentation, record-keeping, and communication.
• Ability to interact with diverse stakeholders (staff, volunteers, next-of-kin and older adults).
• Good communications skills, preferably bilingual with dialects.

Responsibilities

• In-charge of Home Personal Care+/Home Personal Care administrative duties including admission and discharge of referrals and coordination, documentation, finance documentation, subvention/claim submissions and reports, monthly invoicing and payments.
• Maintain records of clients and accounting records for audit purposes; ensure timely reports to government agencies and stakeholders.
• Manage the daily schedule of Senior Healthcare Assistants/Healthcare Assistants and monitor service delivery.
• Obtain periodic feedback from clients/caregivers.
• Carry out monthly means-testing checks on all active clients for Government subsidy eligibility and update records accordingly.
• Outreach to community partners and stakeholders to collaborate on services; e-referral to AIC if required.
• Undertake any other duties assigned by Senior Nurse Manager or Nurse Manager.
