Job summary

WECRUIT PTE. LTD. is seeking a Social Work & Case Management Officer to support residents with tailored care plans and ongoing monitoring of progress within our facility in Singapore.

The role involves developing and coordinating programmes to enhance residents' welfare, conducting six-monthly reviews, and monthly interviews for around 50 residents. Applicants should hold a relevant degree and be prepared for shift rotations.

Qualifications

• Degree or Graduate Diploma in Social Work, Psychology or Counselling.
• Will adhere to established SOPs when carrying out duties.
• Will work rotating shifts according to duty roster.
• Minimum 1 year of relevant work experience.

Responsibilities

• Develop individualised care plans based on residents' needs and conditions.
• Implement care plans and monitor residents' progress and care outcomes.
• Develop and coordinate programmes and activities to support residents' care and well-being needs.
• Prepare six-monthly care review reports and conduct monthly interviews for approximately 50 residents.
