Job summary

Qualifications

• Bachelor’s degree in social work or related field.
• Minimum 2 years’ experience in community or healthcare settings in Singapore.
• Strong communication and stakeholder management skills.

Responsibilities

• Manage case referrals
• Consult with centre lead, conduct assessment, planning and intervention for cases
• Monitor delivery of coordinated services
• Adopt a suitable case management approach when working with clients
• Refer and coordinate care services for clients to ensure they are adequately supported
• Keep abreast and provide information on the various assistance schemes or care options available
• Assess family financial status
• Manage administration of grants, subsidies and support services where relevant or necessary.
• Ensure that all MOH service requirements on financial and quality aspects are adhered to
