Job summary

Recruit Express Pte Ltd invites applications for a Health Coach to support residents in adopting healthier lifestyles and managing chronic conditions. You will coordinate with the Community Health Team to deliver personalised coaching and evidence-based behaviour change strategies.

The role requires engagement across Active Ageing Centres and community sites, with occasional weekend events and strong communication skills in a community health setting.

Qualifications

• GCE A Levels or Diploma and above.
• Diploma in Nutrition, Dietetics, Health Science or related disciplines.

Responsibilities

• Conduct health and lifestyle assessments to identify residents’ health needs and risk factors.
• Engage residents in health discussions and goal-setting using the SMART framework.
• Develop personalised health coaching plans based on wellness goals.
• Guide residents in overcoming lifestyle and behavioural challenges.
• Monitor and document residents’ progress and health outcomes.
• Coordinate holistic care plans with the Community Health Team.
• Travel to Active Ageing Centres and community locations to conduct programmes.
• Support weekend community events when required.
