Responsibilities

• Perform health screening and identify health needs of residents.
• Develop personalised health plans to coach residents toward goals.
• Address challenges to promote healthy behaviours and lifestyle.
• Document progress and maintain accurate resident records.
• Escalate care gaps to community nurses as needed.
• Collaborate with nurses/managers to support health plans.
• Assist residents in navigating social and health systems.
• Escalate complex needs following the escalation matrix.
• Provide basic health education for wellness and prevention.
• Participate in health promotion events and activities.
• Support residents in following-up with their health plan.
