Job summary

NHG Population Health seeks a compassionate health coach to conduct comprehensive care assessments and engage residents in health goal-setting. Qualifications

• Diploma or advanced diploma in a healthcare-related field.
• Strong communication and social engagement skills with older adults.

• Proficiency with computer applications, including Microsoft Office.

Responsibilities

• Assess residents' health status and risk factors through subjective and objective evaluation.
• Facilitate collaborative goal-setting with residents and stakeholders using SMART framework.
• Design tailored health coaching plans to guide residents toward wellness goals.
• Support residents in addressing lifestyle and behavioural change challenges.
• Track and document resident progress and health outcomes.
