Job summary

NTUC Health Co-operative Limited (NTUC Health) invites applications for a Case Executive within the CREST-PDS team to support dementia care clients and their caregivers. You will perform home visits, coordinate services, and contribute to multidisciplinary discussions and documentation.

Ideal candidates hold a Degree/Diploma in Social Work, Psychology or Counselling, have geriatrics knowledge, and demonstrate strong communication, stakeholder management and independent working abilities.

Qualifications

• Degree/Diploma in Social Work, Psychology or Counselling.
• Knowledge of geriatrics and community services.
• Strong communication and stakeholder management skills.
• Able to work independently and be meticulous.
• Proficient with Google Suite.

Responsibilities

• Provide case management support to clients who are newly diagnosed with dementia and their caregivers.
• Provide care coordination and service linkage to clients with dementia and their caregivers, with the appropriate resources and support including caregiver support.
• Participate in multi-disciplinary case discussions with hospital professionals relating to the well-being of clients who are diagnosed with dementia.
• Conduct regular home visits to monitor the well-being of the caregiver and client with dementia.
• Support the evaluation and documentation of the programme.
• Support other ad-hoc assignments as assigned by CREST-PDS Lead/Department Head.
