Job summary

NUHS invites applications for a Care Coordinator (Care Manager) at Alexandra Hospital. The role centers on case management and care coordination across seven specialized programs, supporting clinical leads to ensure holistic patient care.

You will collaborate with multidisciplinary teams, community partners and government agencies to review care models, coordinate workflows and advocate for patient-centered planning.

Qualifications

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health.
• Those with nursing or case management or related field in acute and/or community setting in Singapore is preferred.
• Minimally 5 years of work experience in healthcare industry.

Responsibilities

• Clinical Programme Liaison for case management and care coordination.
• Work with supervisors and functional counterparts to execute care models and workflows.
• Advocate for case management and care coordination to drive programme and hospital objectives.
• Collaborate with Ops and other stakeholders to optimise care manager workflows.
• Coordinate care transitions and ensure documentation and follow-up within the care team.
