Job summary

National University Health System PTE. LTD. is seeking a Care Coordinator to oversee case management and care coordination for patients across Alexandra Hospital’s seven programs.

You will collaborate with the Clinical Director and programme leads to ensure seamless patient care within a multidisciplinary team. The role requires clinical qualifications and at least five years in healthcare, with strong communication and community liaison skills.

Qualifications

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health.
• Minimally 5 years of work experience in healthcare industry.
• Experience working with clinicians and care teams is advantageous.
• Familiarity with hospital systems and documentation practices.

Responsibilities

• Coordinate care models and workflows with clinical teams.
• Advocate for case management to support hospital objectives.
• Triage referrals and assign patients to Care Manager.
• Maintain accurate documentation and ensure timely follow-up on transitions.
• Collaborate with internal and external healthcare providers and community partners.
• Support professional development and department quality improvement efforts.
