National University Health System (NUHS) invites applications for a Programme Coordinator at NUHS@Home. You will coordinate and manage patient care, liaise with clinicians, departments, and external vendors to ensure seamless service delivery in a dynamic home hospital setting.

Qualifications

• Diploma in any discipline.
• Able to converse in English and a mother tongue language (Chinese/Malay/Tamil).
• Proficient in Microsoft Office applications.

Responsibilities

• Involve in the planning, assessment, and monitoring of the program to ensure best possible care.
• Coordinate with external vendors to arrange transport and courier deliveries.
• Coordinate with NUH & cross hospital BMU to manage bed requests.
• Coordinate with multi departments across NUHS to schedule appointments and other services as requested by clinicians.
• Perform patient admissions, discharges and transfers on the EMR system.
• Conduct financial counselling on NUHS@Home bills, subsidies, payment schemes and modes.
