Qualifications

• Minimum 5 years of work experience in healthcare.

• Strong team-player with excellent interpersonal and relational skills.
• Good verbal and written communication; ability to use local languages and dialects is an advantage.

Responsibilities

• Coordinate care models and workflows with supervisors and clinical counterparts.
• Advocate for case management and care coordination to support hospital objectives.
• Collaborate with operations and other stakeholders to optimize programmes.
• Support transitional care and community services discussions with patients and caregivers.
• Ensure timely follow-up and accurate documentation of care activities.
• Participate in case discussions to share updates and best practices.

Education
