Qualifications

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health.
• 6–8 years of healthcare experience; leadership experience preferred.
• Experience collaborating with clinicians and care teams in acute or community settings.
• Familiarity with hospital systems and patient care processes in Singapore is advantageous.

Responsibilities

• Lead case management and care coordination with Clinical Directors to develop care models and workflows.
• Advocate for case management as a driver for programme objectives.
• Coordinate with operations to track outcomes and streamline processes.
• Collaborate with stakeholders and care teams to integrate workflows and optimize patient experience.
• Provide supervision to Care Coordinators and Care Coordinator Associates.
• Engage in professional development and community care collaborations.
• Maintain accurate documentation and participate in quality improvement efforts.
• Support hospital and community events to raise awareness.
• Any other duties as assigned.
