Job summary

National University Polyclinics is seeking a Care Coordinator to support case management and care coordination across the service area. The role emphasizes collaboration with clinical leads, community partners, and the care team to ensure holistic patient care.

The incumbent will advocate for integrated care, coordinate referrals, and contribute to programme improvements while ensuring timely follow-ups and documentation. A nursing or allied health background is preferred.

Qualifications

• Degree or equivalent professional qualification in Nursing, Social Work or Allied Health.
• Experience in hospital/clinical settings preferred.

Responsibilities

• Collaborate with supervisor and counterparts to implement care models and workflows.
• Advocate for case management and care coordination to drive programme and hospital objectives.
• Coordinate with Ops to improve programmes and integrate care workflows.
• Collaborate with care team to optimise patient experience and care manager work.
• Understand patient medical, psycho-social and functional needs with the multidisciplinary team.
• Initiate conversations to identify care gaps and promote healthy lifestyle changes.
• Advocate for advance care planning and discuss with patients and caregivers.
• Refer to transitional care and community services in discussion with patients and caregivers.
• Follow up on patients to ensure smooth care transitions and accurate documentation.
• Participate in case discussions and sharing sessions for learning.
• Triage referrals using hospital channels and update relevant databases.
• Provide supervision and support to Care Coordinators and Associates.
• Contribute to professional development, quality improvement and education activities.
