Job summary

National University Polyclinics is seeking an Assistant / Care Coordinator on a contract to support a multidisciplinary care team in delivering holistic patient care. You will assist in coordinating care activities, track clinical indicators, and use IT tools to document care provided.

Qualified candidates are expected to communicate effectively in English and at least one local language, possess good interpersonal skills, and work well in a team environment. Healthcare exposure is preferred.

Qualifications

• Experience in healthcare preferred.
• Good verbal and written communication skills.
• Pleasant disposition and team player attitude.

Responsibilities

• Coordinate and provide patient care activities using IT systems to document care.
• Track clinical indicators and close gaps in preventive care and vaccination where applicable.
• Assist in coordinating care plans and liaise with care teams and patients/caregivers.
• Provide basic health education to improve health literacy and access to resources.
