DCC Consultants is seeking a data coordination professional to manage the DCC Clinicaldashboard. You will oversee Hospital, Modality Education, Discharge and Missed Treatment reports, ensuring accuracy across GAIA, Ascend and EMR integrations.

The role involves night shifts and collaboration with social workers for transportation issues. Ideal candidates have 3+ years in patient coordination or healthcare settings, and hold a Bachelor’s or Master’s in a related field.

Aufgaben

• Manage data on the DCC Clinicaldashboard.
• Focus on Hospital Report: ensure daily updates and follow up on missing information after 7 days of hospitalization.
• Archive records for Modality Option Education after 6 months.
• Monthly cleanup of reports at the beginning of each month.
• Verify discharges across GAIA and Ascend, and removal in Share source for PD/HHD where applicable.
• Cross-check Missed Treatment data with EMR and coordinate transportation support with social workers.

Job Description

• Responsible for managing the data on the DCC Clinicaldashboard.
• Responsible to focus on the following dashboard reports:
• Hospital Report:To be checked daily. Any patients not entered,the clinic manager is contacted and asked to enter any missinginformation. After 7 days of hospitalisation the clinic mustupdate on the patient’s status as well as updating the dashboard report.
• Modality Option Education Report: Any patients that have beenlisted for 6 months will be archived.
• Report will be cleaned up at the beginning of every month.
• Discharge/Modality Change/Transfer Report: Once patients aredischarged verification will be made that they are discharged fromGAIA and Ascend as well. If PD, verification of patientremoval in Share source is made. If HHD, verification ofremoval from NxStage is made. Discharge report pulled from GAIAand compared to dashboard to ensure all patients are listed.
• Missed Treatment Report: Integrity of information is comparedwith EMR. Follow up is done on patients who haveTransportation issues by contacting the social worker forassistance.
• Dialysis Infection Tracking Report: Comparison to Ascend andGAIA reports will be utilised to ensure data on dashboard is accurate.
• Other reports will be added as needed.
• Other duties, as assigned

Requirements

Education

• Bachelor’s orMaster’s degree in HealthcareAdministration, Nursing, Public Health, or relatedfield.
