Job summary

Remote Raven is seeking an RPM Care Coordinator to translate remote patient monitoring data into timely, compassionate patient outreach. You will review CPAP usage, identify at-risk patients, and document interactions accurately within the EHR and monitoring platform.

The role emphasizes collaboration with clinical, equipment, and scheduling teams, ensuring patients stay on therapy and progress. Training is provided, with a target start by November 1, 2026.

Qualifications

• Minimum 1 year in remote patient monitoring or chronic care management.
• Strong CPAP therapy knowledge and ability to troubleshoot mask fit, pressure, leaks.
• Excellent documentation and HIPAA compliance in a remote setting.
• Calm, encouraging phone presence with patient motivation.

Responsibilities

• Review patients' CPAP usage data daily and flag concerns.
• Make outbound calls (approx. 60/day) to encourage and troubleshoot.
• Document calls in monitoring platform and EHR using templates.
• Coordinate with DME, CPAP supply, and scheduling teams.
• Escalate clinical concerns promptly to providers.
