Job summary

Phoenix Virtual Solutions is seeking an experienced RPM/CCM nurse to support remote patient monitoring and chronic care management programs. You will monitor readings, update care plans, and escalate concerns to clinical staff, while maintaining accurate records in line with HIPAA and privacy requirements.

The role requires 5+ years of hands-on RPM/CCM experience, a PhRN, and strong English communication skills, with the ability to work independently in a remote healthcare setting and

Qualifications

• At least 5 years of hands-on RPM and CCM experience.
• PhRN required.
• Experience with RPM/CCM modules in eClinicalWorks (eCW).
• Knowledge of HIPAA and patient confidentiality requirements.

Responsibilities

• Monitor patient readings and health data, identify abnormal readings, and escalate to clinical staff according to protocols.
• Support ongoing care coordination for chronic conditions, including follow-ups and care plan updates.
• Use RPM/CCM modules in eClinicalWorks to manage patient records, monitoring activities, care plans, and program documentation.
• Contact patients for routine follow-ups, monitoring compliance, care plan reviews, and RPM/CCM participation assistance.
• Maintain accurate, timely, and complete patient records in line with privacy requirements.
• Communicate with providers and clinical teams regarding patient updates and escalations.
• Track patient engagement, monitoring compliance, and completion of RPM/CCM activities.
• Assist with organized workflows and ensure documentation supports billing and program requirements.
