Private Advertiser is seeking a licensed registered nurse to support a US care management program in a fully remote role from the Philippines. You will conduct scheduled calls with US patients enrolled in remote patient monitoring and chronic care management, focusing on routine follow-ups and basic questions.

You will document call notes in the EHR and escalate any clinical concerns to the US care team. The ideal candidate has an active PRC license, 18 months of hands-on clinical work, 12–18

About the role

This is a coordination role supporting a US care management program. You will call patients enrolled in remote patient monitoring (RPM) and chronic care management (CCM) programs, mostly older adults with chronic conditions. You support the US clinical team and do not triage, assess or diagnose.

Key responsibilities

• Make scheduled outreach calls to US patients

• Follow up on blood pressure, glucose and other readings that come in out of range: ask for a second reading and report it to the US clinical team

• Help patients stay on track with their program and answer basic questions about it

• Write clear call notes in the patient record after every call

• Pass any clinical concern to the US care team right away

About you

• Registered nurse with an active Philippine license (PRC)

• At least 18 months of hands-on clinical work

• At least 12 to 18 months of phone experience with US patients, at a US practice via remote work or a healthcare BPO

• Clear, warm spoken English

• A laptop or desktop, a main internet line and a backup line, and backup power

• Able to work US daytime hours

• 2 professional references, 1 of them a supervisor or manager
