Job summary

MEDCOR DASMARIÑAS HOSPITAL AND MEDICAL CENTER INC. seeks a dedicated social worker to conduct psychosocial assessments, provide counseling, and assist indigent patients with financial aid from government and external partners.

The role involves coordinating with clinicians, planning patient care and discharge, referring to community services, maintaining records, educating patients on rights and procedures, and ensuring confidentiality.

Responsibilities

• Conduct psychosocial assessments of patients and families to identify social, emotional, or financial concerns.
• Provide counseling, crisis intervention, and support services for patients facing trauma, terminal illness, or long-term hospitalization.
• Assist indigent patients in availing financial aid from PCSO, DSWD, LGU, PhilHealth, and other external partners.
• Coordinate with physicians, nurses, and other healthcare staff in planning patient care and discharge.
• Refer patients to community-based services, rehabilitation programs, or institutional support when necessary.
• Maintain updated case records, reports, and documentation in compliance with hospital and DOH requirements.
• Participate in hospital committees and contribute to patient welfare programs.
• Educate patients and families about rights, services, and procedures related to care and assistance.
• Ensure confidentiality and ethical handling of sensitive patient information.
• Assist in the preparation of reports for internal monitoring and external audits (e.g., DOH licensing or ISO accreditation).
