Job summary

Responsibilities

• Conduct thorough assessments of patients' emotional, social, and financial needs.
• Provide counseling to patients and families to help them cope with illness, hospitalization, and other related challenges.
• Collaborate with medical staff to develop and implement effective discharge plans.
• Ensure patients have access to necessary post-hospital care, including home healthcare, rehabilitation, and community resources.
• Connect patients and families with community resources such as financial assistance, housing, food, and transportation
• Advocate for patients' needs and rights within the healthcare system and the broader community.
• Respond to emergency situations where immediate psychosocial support is needed.
• Provide interventions in cases of abuse, neglect, or domestic violence.
• Serve as a liaison between patients, families, and the healthcare team.
• Maintain accurate and up-to-date records of all patient interactions and services provided.
• Prepare reports as required by the hospital administration or government regulations.
• Educate patients and families about healthcare processes, rights, and available resources.
• Participate in training programs to stay current with social work practices and healthcare policies.
• Work closely with doctors, nurses, and other healthcare professionals to ensure a holistic approach to patient care.
• Participate in case conferences and multidisciplinary team meetings
• Perform other duties that may be assigned as needed.
