United Antipolo Specialists Hospital and Medical Center seeks a dedicated Social Worker to assess patients’ emotional, social, and financial needs and provide counseling to patients and families. You will collaborate with medical staff to develop discharge plans and ensure access to post-hospital care and community resources.

You will advocate for patients’ rights, respond to emergencies, connect families with resources, and maintain thorough records while coordinating with the healthcare team.

Responsabilités

• Conduct thorough assessments of patients' emotional, social, and financial needs.
• Provide counseling to patients and families to help them cope with illness, hospitalization, and other related challenges.
• Collaborate with medical staff to develop and implement effective discharge plans.
• Ensure patients have access to necessary post-hospital care, including home healthcare, rehabilitation, and community resources.
• Connect patients and families with community resources such as financial assistance, housing, food, and transportation
• Advocate for patients' needs and rights within the healthcare system and the broader community.
• Respond to emergency situations where immediate psychosocial support is needed.
• Provide interventions in cases of abuse, neglect, or domestic violence.
• Serve as a liaison between patients, families, and the healthcare team.
• Maintain accurate and up-to-date records of all patient interactions and services provided.
• Prepare reports as required by the hospital administration or government regulations.
• Educate patients and families about healthcare processes, rights, and available resources.
• Participate in training programs to stay current with social work practices and healthcare policies.
• Work closely with doctors, nurses, and other healthcare professionals to ensure a holistic approach to patient care.
• Participate in case conferences and multidisciplinary team meetings
• Perform other duties that may be assigned as needed.
