Brief Job Summary

• Provide psychosocial assessment and support to hospital patients and families
• Assess social, emotional, and financial needs affecting patient care
• Facilitate access to DSWD, LGU, PhilHealth, PCSO, and NGO assistance programs
• Prepare and submit Social Case Study Reports and required documentation
• Support discharge planning and continuity of patient care
• Work collaboratively with the multidisciplinary healthcare team
