The Medical Social Worker is responsible to provide professional care to those who are in especially difficult circumstance, responsive to the psycho-social needs of the vulnerable patient and their families.
DUTIES AND RESPONSIBILITIES:
- Conduct psychosocial assessment and classify patients according to their economic status and other health needs.
- Provide psychosocial casework to prevent social breakdown by restoring social functioning of the patient.
- Provide counseling to patients in need.
- Conduct a multi-disciplinary team consultation and attend case conferences.
- Establish linkages to funding agencies, and provide referrals and networking.
- Maintain records and documentation of social work services.
