Job summary

CF Outsourcing Solutions is seeking a Patient Engagement Campaign Coordinator to plan, execute, optimize, and monitor campaigns that drive enrollment into Care Management programs. This role bridges Operations, Marketing, Clinical Teams, and vendor partners to ensure outreach is timely and effective.

You will develop strategies, determine timing and channels, analyze performance, and create messaging that improves conversion and enrollment rates, while ensuring patient-centered and compliant

Qualifications

• Bachelor's degree in Marketing, Healthcare Administration, Business, Public Health, or related field.
• 2+ years of experience in healthcare operations, patient engagement, or campaign management.
• Knowledge of CCM, RPM, and population health initiatives.
• Familiarity with patient engagement best practices and digital outreach campaigns.

Responsibilities

• Develop and implement patient engagement campaigns to increase enrollment into Care Management programs.
• Determine best outreach methods: SMS/Email campaigns, call campaigns, app notifications, multichannel strategies.
• Create campaign calendars, schedules, and cadence to maximize response and enrollment.
• Monitor conversion and enrollment performance; analyze metrics and identify barriers.
• Collaborate with Operations Leadership and vendor partners; prepare reports on reach and enrollment.
