Job summary

ALLIED SEARCH PTE. LTD. is seeking an Executive for Eldercare Services in a contract/community setting to drive programme implementation and care coordination.

The role requires conducting interRAI assessments and building partnerships with healthcare and social service organisations within sub-regions. The successful candidate will coordinate meetings, monitor progress, and address gaps in service delivery, while contributing to management reporting and programme oversight.

Qualifications

• Possess a degree in Social Work, Nursing or healthcare-related discipline.

Responsibilities

• Drive the implementation of programmes and workplans, including integrated care initiatives, joint referral workflows, and care coordination activities.
• Conduct interRAI assessments and support care planning and coordination for clients, where required.
• Build and manage strong partnerships with healthcare, community and social service organisations within assigned sub-regions to facilitate service integration.
• Represent the organisation in the sub-regional initiatives and serve as a key liaison between internal teams, external partners, and community stakeholders.
• Coordinate meetings and operational activities, monitor programme performance and progress, and support the resolution of service or operational gaps.
• Identify opportunities to improve service integration and quality, while supporting management reporting, dashboards, presentations, and programme oversight.
• Any other ad hoc duties as assigned.
