Job summary

Easterseals Midwest in St. Joseph, MO is seeking a dedicated Community Living Specialist to support individuals with disabilities in their homes and community. This part-time role offers growth opportunities, education, and a supportive team environment committed to inclusive living.

Responsibilities include ISP implementation, daily living supports, transportation, and coordination with medical teams. Benefits include PTO and health coverage for full-time staff; drivers license is required.

Qualifications

• Must have high school diploma or equivalent.
• Experience providing direct supports to individuals with disabilities preferred.
• Must have basic computer skills and be able to work with a variety of computer systems.
• Driver's license, car insurance and reliable transportation are required.

Responsibilities

• Reviews and implements activities related to Individual Support Plans (ISPs) and works towards plan outcomes and objectives.
• Provides education, assistance, mentoring and support to individuals; supports may include daily living activities.
• Provides assistance during community outings and social opportunities.
• Administers medications and provides medical care per physician orders and RN delegations; accompanies individuals to medical appointments.
• Provides positive behavioral interventions as outlined in ISP or behavior development plan.
• Attends all required staff meetings and opportunities for ongoing education and training.
