Job summary

The role provides supervision of assigned staff, 1:1 consumer support, and participation in Circle of Support and Person Centered Planning processes, while advocating for consumer rights and safety within the community.

Qualifications

• Bachelor’s degree plus 3 years experience in rehabilitation or social work.
• Ability to implement ISP objectives and advocate for client rights.
• Knowledge of housing market and transportation systems.
• Health and safety considerations in community settings.

Responsibilities

• Staffing through scheduling, recruitment, hiring, training and orientation of direct service, substitute and volunteer staff.
• Provide 1:1 support in the consumer’s home and community as identified in their Personal Plan.
• Assist and participate in the Circle of Support and implementation of the Person Centered Planning Process.
• Facilitate and develop Circle of Support to build relationships with client, family, friends, neighbors, SARC, clients, relief staff, and roommate.
• Development and expansion of consumer participation in community activities and outreach to work.
• Complete required forms and documentation including incident reports and billing.
• Assist consumers with housing and training needs.
• Maintain all assigned client case records and billing records.
• Maintain professional relationships with consumers, family, doctors and others; represent HOPE in the community.
• Act as a mandated abuse reporter.
• Perform other duties to assure program efficiency.
