Job summary

North Bay Regional Center is seeking a Social Services Case Manager to join our team of Service Coordinators. You will manage service delivery for clients, ensuring IPP goals are pursued and resources are optimally used to support individuals with developmental disabilities.

Responsibilities include using person-centered planning, coordinating community resources, and performing annual reassessments with an interdisciplinary team.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities (e.g. Social Work, Psychology, Counseling, Special Education, Nursing).
• Master’s Degree preferred.
• 3 years’ experience working with individuals with developmental disability.
• Bilingual language ability (Spanish to English) is preferred.

Responsibilities

• Manage delivery of services to regional center clients including those with intensive needs.
• Ensure Individual Program Plan objectives are pursued and aligned with client goals.
• Use person-centered planning to help clients access resources before using regional center funds.
• Conduct annual reassessments and occasional client assessments with the interdisciplinary team.
