Job summary

Digital Minds BPO seeks a Care Navigator (Clinical Background) to support mothers through a US-based digital maternal health platform, using clinical knowledge to guide care and follow-up with patients.

You will document activities in client systems, ensure privacy, and coordinate care according to navigation protocols while onsite in Naga City, Philippines.

Qualifications

• Active PRC License as RN or RM; clinical background required.
• Demonstrated years of patient care experience; license alone not enough.
• Strong English communication; confident patient interactions by phone.
• Excellent interpersonal skills, compassion and professionalism.

Responsibilities

• Provide personalized care navigation to pregnant and postpartum mothers.

Position Overview

Join our growing team and support pregnant and postpartum mothers through a leading U.S.-based digital maternal health platform. As a Care Navigator (Clinical Background), you will leverage your clinical knowledge and hands-on healthcare experience to provide compassionate patient support, guide mothers throughout their maternal health journey, conduct follow-ups, and help coordinate care while working within established navigation protocols.

What You'll Do

• Provide personalized care navigation and support to pregnant and postpartum mothers.
• Apply your clinical judgment to patient questions while remaining within the scope of the Care Navigator role.
• Conduct patient follow-ups through phone calls and messaging to monitor patient needs and engagement.
• Accurately document all patient interactions and care activities within the client's systems.
• Ensure strict compliance with HIPAA regulations and maintain patient confidentiality at all times.

Required Qualifications

• Active PRC License as a Registered Nurse (RN) or Registered Midwife (RM).
• Demonstrated clinical experience with actual years of patient care practice. A PRC license alone does not qualify.
• Strong English communication skills with the ability to confidently and empathetically communicate with patients over the phone.
• Excellent interpersonal skills, compassion, and professionalism when assisting patients.
• Willing to work onsite in Naga City, Bicol.

Preferred Qualifications

• Experience working with pregnant and postpartum patients.
• Experience in maternal health, women's health, community health, telehealth, or U.S. healthcare environments is an advantage.
• Experience with patient documentation and digital healthcare platforms is a plus.
