We need a Nursing or Allied Health Clinician to join ourAdult Community and Long-Term Conditions Servicebased at Te Toka Tumai - Auckland. As service co-ordinators, our team is committed to delivering support services to people whose health needs need to be met at home in a locality-based framework. Our community team provides assessment and service coordination to adults who have long term disabilities or chronic long-term health conditions.
