Job summary

Health New Zealand – Te Whatu Ora in Auckland is seeking a Nursing or Allied Health Clinician to join the Adult Community and Long-Term Conditions Service. The Service Coordinator delivers assessment and service coordination for adults with long-term disabilities or chronic conditions, working in home-based care within a multidisciplinary team.

You will hold NZ registration with a current annual practising certificate, a full NZ driver’s licence, and preferably community-care experience.

Qualifications

  • NZ registered health professional with current annual practising certificate.
  • Clean full NZ drivers licence.
  • Experience in community setting is desirable.
  • Commitment to development and implementation of quality improvement initiatives.

Responsibilities

  • Assess and coordinate services for adults who need home-based care.
  • Collaborate with multidisciplinary teams to plan patient-centred care.
  • Provide needs assessment and service coordination in a locality-based framework.
  • Support ongoing professional development and learning within the team.

About the Role

We need a Nursing or Allied Health Clinician to join our Adult Community and Long-Term Conditions Service based at Te Toka Tumai - Auckland. As service co-ordinators, our team is committed to delivering support services to people whose health needs need to be met at home in a locality-based framework. Our community team provides assessment and service coordination to adults who have long term disabilities or chronic long-term health conditions.
