Job summary

Health New Zealand | Te Whatu Ora in Auckland is seeking a Registered Nurse or Allied Health Clinician to join our Adult Community and Long‑Term Conditions Service at Te Toka Tumai. As a service co-ordinator, you will deliver home-based assessments and coordinate care for adults with long-term disabilities or chronic conditions, backed by a multidisciplinary team.

  • Deliver care coordination within a locality-based framework as part of a multidisciplinary team.

We need a Nursing or Allied Health Clinician to join our Adult Community and Long‑Term Conditions Service based at Te Toka Tumai - Auckland. As service co‑ordinators, our team is committed to delivering support services to people whose health needs need to be met at home in a locality‑based framework. Our community team provides assessment and service coordination to adults who have long term disabilities or chronic long‑term health conditions.
