Job summary

Hawkes Bay DHB in Napier is seeking a Registered Nurse or Allied Health professional to join our Needs Assessment and Service Coordination team.

Qualifications

About the Role

We are seeking either a Registered Nurse or Allied Health professional to join our Needs Assessment and Service Coordination (NASC) team. In this rewarding community-based role, you will work alongside older people and those living with long-term health conditions to help identify their needs and outline what support services are available to support services that enable them to live safely and independently in their homes.

Working closely with clients, whānau, healthcare providers, and community organisations, you will undertake a comprehensive clinical assessment, develop care plans, make recommendations for support services. This role offers the opportunity to build meaningful relationships, advocate for equitable access to services, and make a genuine difference in the lives of people across Hawke's Bay.

About the Team/Service/ Location

Needs Assessment Service Co-ordination Hawkes Bay is a Service within the Integrated Care Team, Specialist and Community Services Group. Integration services working seamlessly together creating pathways with clients and whanau at the centre

The NASC Hawke's Bay team provides needs assessment and service coordination for older persons and people living with long-term health conditions.

Based in Napier, you'll enjoy a role that combines office-based work with community engagement, while living in one of New Zealand's most sought-after regions, renowned for its sunny climate, coastal lifestyle, and vibrant community.

About you

We welcome applications from either Registered Nurses or Allied Health professionals who bring:

  • Current New Zealand professional registration and a current Annual Practising Certificate.
  • Strong assessment, care planning, communication, and relationship-building skills.
  • A commitment to Te Tiriti o Waitangi and improving health outcomes for Māori.
  • Excellent time management skills and the ability to work both independently and collaboratively.
  • A passion for supporting older people and those with complex health needs to achieve the best possible quality of life.
  • Experience in needs assessment, service coordination, community health, or care planning.
  • Working knowledge of InterRAI.
  • Experience working with older persons and/or people with complex health and social needs.
