Job summary

Health New Zealand | Te Whatu Ora Lakes seeks a Needs Assessment Service Coordinator to join our Taupo-based team. You will perform holistic assessments and coordinate support for older adults and others with long-term conditions, working with clients across the Lakes region including Rotorua, Mangakino, Taupo and Turangi.

The role requires NZ Registered health professionals (Social Worker, OT, Physiotherapist or Nurse) with current practicing certificates, strong communication, and

An exciting opportunity has become available to work as a Needs Assessor Service Coordinator and join our passionate and multi-disciplinary team at Lakes NASC based in Taupo.

About the role: Our work is fast paced and dynamic. We offer a positive and supportive team environment which is responsive to clients needs as well as enabling professional opportunities for learning and development. The role of NASC is to complete holistic assessments of the needs of our clients, and coordinate support services for older adults with health and disability related needs, and some younger people whose needs relate to a chronic long-term health condition. We also work with clients who have recently discharged from hospital, as well as palliative care clients. Our aim is to support our clients to continue to live safely at home, or enable them to move into Residential Care if needed.

This role involves working with clients across the Lakes region (Rotorua, Mangakino, Taupo, Turangi).

About you:

  • New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist or Nurse
  • Current New Zealand Annual Practicing Certificate in appropriate scope
  • Allied Health or Nursing qualification and relevant professional experience
  • Full NZ Drivers License

You will have proven assessment and clinical reasoning skills. In addition, you will have experience of working in partnership with clients and their whanau to meet their support needs. You will have excellent communication skills, as well as a record of positive relationship building. Your strengths will include your resilience, compassion and commitment to positive outcomes for our people.
