  • Provide needs assessment and service coordination for adults with long-term disabilities or chronic health conditions in home-based settings.
  • Deliver assessment and coordination of services in a locality-based framework with a multidisciplinary team.
  • Develop and implement care plans in collaboration with clients and families.
  • Participate in orientation and ongoing professional development.
