Job summary

Third Age Health Services Ltd is seeking a passionate Registered Nurse to provide high-quality clinical care across aged residential care facilities and GP clinic services in Auckland.

The role is hands-on with autonomy and purpose as Clinical Coordinator, delivering direct patient care during facility visits and supporting GP clinic services. You will drive quality improvement and coordinate care across ARC facilities.

Qualifications

  • Must be a Registered Nurse with APC and CPR certification.
  • Experience in aged care or primary care settings is essential.
  • Full or restricted driver’s license required.

Responsibilities

  • Provide practice nursing support within GP clinic and ARC facilities.
  • Conduct facility visits for clinical procedures (IV infusions, ear suctioning, liquid nitrogen, ECGs).
  • Support on-call team with telephone triage and clinical advice.
  • Contribute to quality improvement activities and audits.
  • Coordinate care with facilities, clinicians, PHOs, ACC and internal teams.

Third Age Health (TAH) is seeking a passionate and experienced Registered Nurse to provide high-quality clinical care across our aged residential care facilities and GP clinic services.

TAH is New Zealand's leading provider of general practice medical services in Aged Residential Care (ARC). We champion high-quality care for older adults and support our teams to deliver excellent clinical outcomes.

The Role:

This is a hands-on clinical role with variety, autonomy, and purpose. As Clinical Coordinator, you will spend your time delivering direct patient care through facility visits and supporting our GP clinic services. Working closely with our GPs and aged care facilities, you will help ensure residents receive timely, high-quality clinical care.

Alongside your clinical responsibilities, you will contribute to service improvement initiatives, clinical audits, and quality projects that enhance care delivery across our ARC network.

What you'll do:

Provide practice nursing support within our GP clinic and aged residential care facilities

Conduct regular facility visits across Auckland to deliver clinical procedures including IV infusions, ear suctioning, liquid nitrogen and ECGs

Support our on-call team with telephone triage and clinical advice

Contribute to quality improvement activities, audits, and service development projects

Liaise with facilities, clinicians, PHOs, ACC, and internal teams to ensure seamless care delivery

Who you are:

Registered Nurse with a current APC and CPR certification

Experience in aged care, primary care, general practice, or community nursing

Confident working autonomously across multiple clinical settings, therefore a full or restricted driver’s license is required

Skilled in clinical assessment and care coordination

Organised, adaptable, and committed to continuous quality improvement
