Job summary

Goodwood Park Healthcare Group in West Auckland is seeking a passionate Registered Health Professional (Occupational Therapist, Social Worker, DAPAANZ, RN, Mental Health Nurse) with APC/registration to provide clinical services at our residential sites.

You will conduct clinical assessments, formulate client rehabilitation plans with the multidisciplinary team, deliver individual and group interventions, and collaborate with families, funders, ACC and other agencies.

Qualifications

  • Registered Health Professional with APC/registration in a relevant field (OT, SW, RN, MHN)
  • Minimum 2 years experience in health/rehabilitation fields
  • NZ residency or valid NZ working visa
  • Full NZ driver’s licence and computer literacy

Responsibilities

  • Conduct clinical assessments using evidence-based outcome measures
  • Formulate and implement high-quality rehabilitation plans with the multidisciplinary team
  • Deliver individual and group therapeutic interventions as clinical keyworker
  • Collaborate with clients, families and other stakeholders
  • Provide integrated care with community organisations and service providers
  • Meet contractual KPIs and participate in multidisciplinary meetings and case reviews
  • Coach and support staff with goal planning and intervention

The GoodwoodPark Healthcare Group is a progressive rehabilitation basedorganisationand a leading provider of residential and community support services for people who have a Mental Health illness, Traumatic Brain Injury, Spinal Injury, or dual diagnosis.

The role

We are looking for apassionate Registered Health Professional(Occupational Therapist, Social Worker, DAPAANZ, RN, Mental Health Nurse) with an APC/registration to provide clinical services to our clients based at our residential sitesin West Auckland.

Following an assertive case management framework, you will provide client-centred clinical support and therapeutic rehabilitation to clients to ensure their independence is maximised/maintained, and recovery is optimised to enhance their quality of life and sense of well-being.

A key aspect of the role will be liaising with funders and other agencies including ACC, DHB clinical teams, WINZ, and other community organisations, developing partnerships that ensure our clients are able to achieve their goals.

The role is full-time; predominantly Monday-Friday but may include some weekend hours or evening work once or twice a month depending on clients need. There will be some on-call component to the role, being one week of phone support to our residential services every 3-4 months.

The key tasks of this role include:

Conducting clinical assessments to evidence outcome measures

Formulating and implementing high quality client rehabilitation plans in collaboration with the multidisciplinary team

Delivering individual and group therapeutic interventions as clinical keyworker

Working collaboratively with clients, their families and other stakeholders

Providing integrated care with other community organisations or service providers

Meeting contractual KPI’s,

Participation in multi-disciplinary meetings within the organisation and case reviews with funders or stakeholders

Coaching and supporting staff with goal planning and intervention

Maintaining relationships with Mental Health clinical services and ACC Case Managers

To ensure that the principles of the Treaty of Waitangi are supported and implemented in the delivery of services.

Our ideal candidate for this role will possess the following skills and attributes:

  • A passion for mental health/TBI rehabilitation
  • Driven to foster positive change in clients
  • Able to work to deadlines, demonstrate resilience and maintain a positive attitude
  • Confident with report writing, good time management and great organisational skills
  • Team oriented and a great communicator
  • An annual practising certificate or registration as a health professional in a relevant field (Social Work, Mental Health Nursing, RN, DAPAANZ, Occupational Therapy)
  • Be able to work in a culturally responsive way
  • 2 years experience working in the Health and Disability/rehabilitation fields
  • A full NZ drivers licence
  • Proficient computer literacy – Microsoft suite
  • Have NZ residency /valid NZ working visa
