Job summary

Te Kōhao Health in Kirikiriroa (Hamilton) is seeking a Health Coach for the Takarangi Integrated Primary Mental Health and Addictions team. You will work with individuals and whānau to identify what matters, set achievable goals and build confidence in managing health.

Through practical coaching, follow-up and encouragement you will help whānau develop healthy habits, improve sleep and activity, and connect with local services.

Qualifications

  • Experience in health coaching or behaviour change support.
  • Ability to communicate clearly with diverse communities.
  • Understanding of culturally responsive, whānau-centred practice.

Responsibilities

Health Coach – Takarangi

Help whānau turn their wellbeing goals into everyday change.

Are you someone who connects easily with people, listens without judgement and helps others take positive steps forward? We are looking for a Health Coach to join Takarangi, our Integrated Primary Mental Health and Addictions team at Te Kōhao Health.

Te Kōhao Health is a kaupapa Māori provider committed to supporting the health and wellbeing of whānau. Our approach recognises the connections between physical, mental, spiritual and whānau wellbeing, and places whānau aspirations at the centre of our mahi.

Takarangi brings together health improvement practitioners, health coaches and support workers to help people access support within primary care.

Mō te tūranga | About the role

As a Health Coach, you will work alongside individuals and whānau to identify what matters to them, set achievable goals and build confidence in managing their health and wellbeing.

Through practical coaching, encouragement and follow-up, you will support people to develop healthy habits and make changes they can sustain. This may include improving sleep, increasing physical activity, developing healthy routines and finding ways to manage everyday stress.

You will work closely with the wider Takarangi team and primary care colleagues to ensure support is coordinated and responsive to whānau needs.

Your mahi will include:

  • Building trusting, respectful relationships with individuals and whānau.
  • Supporting whānau to identify their strengths, priorities and wellbeing goals.
  • Breaking goals into manageable steps and reviewing progress together.
  • Helping whānau recognise and work through barriers to positive change.
  • Connecting people with relevant community activities, resources and services.
  • Working collaboratively with health improvement practitioners, support workers and primary care teams.
  • Maintaining accurate, timely records and protecting whānau privacy.
  • Recognising when additional support is needed and involving the appropriate team members.

You bring warmth, patience and a practical approach to supporting people. You understand that meaningful change starts with listening, building trust and working at a pace that suits each person.

We are looking for someone who:

  • Has experience supporting people in health, wellbeing, community or whānau services.
  • Can motivate and encourage others while respecting their choices.
  • Understands tikanga Māori and is committed to culturally responsive, whānau-centred practice.
  • Communicates clearly and builds relationships with people from different backgrounds.
  • Is organised, comfortable using digital systems and able to work independently and within a team.
  • Understands professional boundaries, confidentiality and when to seek guidance.

Experience in health coaching, behaviour change or mental health and addictions would be an advantage.
