The Doctors Drury is seeking a motivated and compassionate Health Coach to join our team and support patients on their journey towards better health outcomes. are based within GP clinics, working alongside GPs, nurses, and the wider primary care team to support patients as part of their everyday care.
You will help patients build the knowledge, skills and confidence they need to manage their own health and wellbeing, while connecting them with the services and support available within their community.
This is a permanent part time role, working 21 hours per week over 3 days across Monday - Friday.
As a core member of the practice team, you will provide support through face-to-face, telephone and virtual consultations. You may also facilitate group programmes, assist patients to navigate health and wellbeing services, and work collaboratively with the wider clinical team to improve engagement and outcomes for patients who may be harder to reach.
We are looking for someone who genuinely enjoys working with people and is passionate about empowering others to improve their health and wellbeing.
