Job summary

Te Arawa Whānau Ora invites applications for a Health Coach to support whānau health and wellbeing through a Mana Māori framework. You will build strengths-based plans, maintain a caseload and collaborate with clinicians and local providers.

Qualifications

Te Arawa Whānau Ora Charitable Trust is a collective of whānau ora providers in the Te Arawa area. Our mission is working collaboratively for whānau-centred outcomes. Within Te Arawa Whānau Ora, we intend to create transformative environments that influence the way we support whānau to achieve mana motuhake. We aim to strengthen whānau, cultivate energies of excellence, and be guided by our tūpuna aspirations as kaitiaki o te taiao. Kia mau ki te aka tāepa, kia mau ki te aka matua.
We are seeking a passionate Health Coach to join our Manu Tāpiki team. As a Whānau Unifier within the Integrated Primary Mental Health and Addiction team, you will walk alongside whānau to support their health and wellbeing aspirations. We deliver from a Kaupapa Māori and values-based framework and welcome applicants who are committed to supporting whānau to strengthen their confidence, capability, connection and mana motuhake.
The Health Coach works in partnership with whānau to understand what matters to them, develop strengths-based plans, set meaningful goals and support progress towards their intended outcomes. You will provide holistic emotional, practical and spiritual support for whānau experiencing mental distress, addiction-related challenges or wider wellbeing concerns. You will help make the health and social system easier to navigate by acting as a bridge between whānau, clinicians, general practices and other support services.
This role includes maintaining a caseload, working collaboratively with the wider team and local providers, delivering monthly prevention and wellbeing workshops, and assisting with whānau, community and health-promotion events. You will maintain timely, accurate documentation, contribute to reporting and continuous improvement, and work in a way that reflects the values and models of practice of Te Arawa Whānau Ora.
