Te Mana Oranga Trust is seeking someone who has had experience working in the mental health and addiction field to join a dedicated General Practice (GP) team in Kaikohe, Northland. Working full-time, Monday to Friday, this role plays a crucial part in supporting patients on their journey to better health and wellbeing. As a Health Coach, you will work closely with individuals to create personalised plans, provide guidance and offer solutions, and empower them to make sustainable lifestyle changes.
Key responsibilities
Use formal assessment tools (Duke or Hua Oranga) to identify, prioritise and set goals to overcome health challenges
Develop individualised coaching plans and strategies to help clients achieve their health objectives
Provide ongoing support, motivation, and accountability to help clients achieve their goals
Work collaboratively in a GP practice as part of a multidisciplinary team of healthcare professionals and assistants to ensure a holistic approach to client care and wellbeing is provided
Maintain detailed client records and documentation daily in accordance with organisational policies and procedures
Transport patients from their homes to appointments in Kaikohe, Whangarei or Kaitaia as the needs arise
Continuously expand your knowledge and skills by attending training and professional development opportunities
About you - What we are looking for
Enthusiastic, vibrant, highly motivated and wants nothing more than to help others in need
Relevant experience in health, wellness, or a related field
Minimum 2 years' experience working in health, wellness or in a similar role
Excellent interpersonal and communication skills, with the ability to build trusting relationships with patients
Team player who can also work independently
Strong problem-solving and critical thinking abilities to tailor coaching strategies to individual needs
Passion for empowering individuals to make positive lifestyle changes and improve their overall health and wellbeing
Familiarity with healthcare systems and community resources in the Northland region
A reliable person who will turn up to work, rather than take days off work here, there, and everywhere, for this, that and the other - FOR REAL!
Reliable transport - although you will use a work car, you must have your own vehicle to get to and from work

Qualification and Experience
Minimum qualification - Level 4 Mental Health and Wellbeing
Relevant experience in mental health, nutrition, or a related field
Experience in health coaching or a similar role, with a proven track record of helping clients achieve their goals
Full driver's licence (Planning to get a Full driver's licence is not a Full Driver's licence)
You might have been working as a Community Support Worker
Basic te reo an advantage
A passion for health and wellness and a commitment to empowering individuals to take control of their health
A team player who collaborates and can work independently
The successful candidate must undergo a Police check
An application for employment and health questionnaire will also be part of the process
The successful applicant must undergo the Health Coach training provided by Prekure (unless you have already completed it), completing all tasks within set deadlines as a condition of being employed as a Health Coach
