Qualifications

• Diploma or degree in Social Work, Community Development, or a related field.
• Proven experience in support coordination, case management, or community services, preferably with vulnerable populations.
• Excellent communication, interpersonal, and active listening skills.
• Proficiency in using telehealth platforms, CRM software, and standard office applications.
• Strong organizational skills and the ability to manage multiple tasks and deadlines in a remote work setting.
• Demonstrated empathy, patience, and a commitment to client advocacy.

Responsibilities

• Coordinate and manage a caseload of vulnerable individuals, ensuring their support needs are met promptly and effectively.
• Liaise with clients via phone, email, and video conferencing to assess needs, provide information, and offer guidance.
• Connect clients with appropriate community resources, such as healthcare, social services, housing assistance, and counseling.
• Monitor client progress and well-being, identifying any potential issues and escalating them to the appropriate team members.
• Maintain accurate and confidential client records within the organization's secure digital systems.
• Advocate for clients to ensure they receive equitable access to necessary services and support.
