Job summary

Health New Zealand | Te Whatu Ora invites applications for the Coordinator - Coasters Home & Community Support Services. This role coordinates care to help clients live well, age well, and maintain dignity in their own communities, working closely with Referrers, Coasters Nurses, and Support Workers.

You will rostering, cover annual leave and training, and ensure culturally safe, high-quality customer service to clients and families.

Qualifications

Proficiency in Microsoft applications for rostering/scheduling.
Ability to work autonomously and within a team.
Commitment to confidentiality and discretion.
Strong interpersonal and communication skills.
Cultural sensitivity and respectful engagement with clients.
Professional presenting image and demeanor.
Ability to work under pressure.

Previous experience in service coordination.
Experience with rostering systems.
Experience in health, community or aged care settings.

Responsibilities

Coordinate service requests with Service Referrers and Coasters Nurses.
Liaise with Support Workers and ACNM/AH lead regarding rostering and leave Coverage.
Provide high-quality service to clients, whanau and families.
Deliver culturally safe, respectful support upholding client dignity (mana).
Participate in training and skills development.
Utilise technology to support administration and communication.
Assist onboarding and skill development of other team members.
