Job summary

Canterbury District Health Board seeks a Coordinator for Coasters Home & Community Support Services on a full-time, 40-hour-per-week basis to coordinate care for clients in their communities. Qualifications

Experience in service coordination role.
Experience in rostering systems.
Experience in health, community or aged care settings.
Competent using Microsoft applications and rostering software.

Responsibilities

Coordinate care to support clients to live well and age well.
Arrange service requests with referrers and nurses daily.
Liaise with support workers and leads on rostering, leave, sickness and training.
Provide high-quality, culturally safe customer service to clients and families.
Support onboarding, training and skill development of team members.
Use technology to support administration and communication.
