We are looking for a full-time Medicine Care Coordinator to join our team at Jenner Health Centre. This is an exciting opportunity to work across Modality Lewisham Division practices as part of a multidisciplinary healthcare team, working closely with the Lead Pharmacist and PCN Manager.

You will play an important role in supporting the delivery of medicines-related services, helping the team achieve key performance indicators and targets while contributing to improvements in patient care, service delivery, and ways of working.

Main duties of the job

As a Medicines Care Coordinator, you will work closely with the Lead Pharmacist and PCN Manager to support the delivery of medicines-related services and achieve key performance indicators and targets outlined within standard operating procedures and the business plan.

This is a dynamic role that requires innovation, flexibility, commitment, and the ability to work resourcefully as part of a multidisciplinary team. You will contribute to the delivery of high-quality services while helping to identify opportunities to improve processes, patient care, and the overall patient experience.

You will work directly with key stakeholders to help harmonise ways of working, improve working practices, and contribute to sustainable improvements across Primary Care.

Key responsibilities include

• Working with the Lead Pharmacist and PCN Manager to support the achievement of KPIs, targets, and performance indicators
• Contributing to the delivery and coordination of medicines-related services across the Division
• Working collaboratively as part of a multidisciplinary healthcare team
• Supporting the implementation of innovative solutions and improved ways of working
• Working with key stakeholders to harmonise processes and improve service delivery
• Contributing to feedback, service improvement, and business planning
• Working sensitively with patients, their families, and carers
• Using initiative and problem-solving skills to identify opportunities for improvement
• Working flexibly and resourcefully to ensure tasks and priorities are completed effectively

£66,274 - £73,496 per annum inclusive of inner HCAs
